Cancellation Policy

ABO Phlebotomy School – Phlebotomy Standard Track Student Agreement Please review and check each box below. ☐ This program includes classroom instruction followed by the NHA Exam. Student must pass the NHA exam in order to be placed on a waiting list for clinical externship. ☐ There are two separate fees not included in the program cost: $130 for the national exam (per attempt) and $100 for the CPT 1 license application fee after graduation. ☐ Passing the national exam is my responsibility. Instruction is provided, but passing is not guaranteed. ☐ Attending class is not enough. I must study outside of class using reading, writing, repetition, self-quizzing, and rewording content. ☐ Clinical dates and locations depend on availability. No preferences or guaranteed start dates. ☐ Urgent openings may be offered with less than 24 hours' notice to help students graduate faster. Declining or missing one may delay placement. I may be moved back in line. I am responsible for being available. ☐ If I find my own site or want to use my job site, it must be approved first. Approval is not guaranteed, may take time, and reasons may not be disclosed. ☐ Zero absences allowed. I’m responsible for making up missed material. ☐ I agree to act professionally at all times. No smoking, chewing gum, phone use, or non-clinical conversation during clinical. No recording of any kind in class or clinic. ☐ I will wear scrubs (any color/style) and closed-toe shoes as required. ☐ I will be on time to all class and clinical days. Tardiness or absence may delay my progress. ☐ All financial obligations must be completed to graduate. ☐ I will take full advantage of all clinical opportunities offered. ☐ If I have a learning disability, I will notify the school early. I am responsible for asking questions and reaching out before or after class or by email. ☐ I meet all enrollment requirements: 18+, high school education, and valid photo ID. I understand I am required to bring a photocopy of my high school or college diploma and a photo ID. ☐ By checking this box, I agree to all school policies listed above and in the program materials.
